The Eden Institute of Plastic and Cosmetic Surgery is a medical practice which at times may collect and hold personal information necessary to carry out activities including communication with our existing and potential clients and interested parties.

As a part of our commitment to providing quality health care, it is necessary for us to maintain files pertaining to you. We respect and uphold your rights to privacy protection under the National Privacy Principles contained in the Privacy Amendment (Private Sector) Act 2000.   • Name, address, telephone number(s) * Date of Birth * Occupation * Health Information * General Practitioner * Referring Doctor * Veterans Affairs Association * Details associated with services we have provided to you * Additional information provided to us by you or referring agencies * Any information you provided to us through client/consumer surveys * Clinical photographs or pre and post-operative procedures
  • With your consent we may use this information, amongst others : * to communicate promotional offers and special events to you * for direct mailing to you * for service planning to you * for research and development for our practice 1.The Eden Institute of Plastic and Cosmetic Surgery will not disclose personal information about individuals to any person except when: You have consented to the disclosure as per the Practice Consent Form 2.The secondary purpose is related to the primary purpose (for which the information was collected) and a person would reasonably expect such use or disclosure; or 3.The use is for direct mailing in specified circumstances: or 4.It is required by Commonwealth/State legislation or in circumstances related to public interest or public or individual health and safety.
  • Individuals may at any time opt out of receiving any communications from The Eden Institute other than as required for the operation of our services.
